2013/11/06 放射線従事者登録の様式変更について

J-PARC放射線障害予防規定が改定され、平成25年11月より施行されましたので、認定登録依頼書兼管理区域立入許可願が変更となりました。


これから登録を行う場合は、新しい様式を共同利用者支援システムよりダウンロードして下さい。
ご理解のほど、よろしくお願い致します。


2013/11/06　A change of Radiation Worker Registration form

Inner rules for the Prevention of Radiation Hazards for the J-PARC has been revised and enforced on November 1st, 2013, so the format of Radiation Worker Registration form was changed.
Please download the new format from the User Support System, if you want to register as a new radiation worker at J-PARC.
