Documents to be submitted

Submit documents at least 2 weeks before the start of your experiment.

Safty Checklist
Submission of Safty checklist for 2016A General Use is no longer required.
Submit “MLF Safety Checklist” by E-mail to the J-PARC Users Office
 
 
a: Letter of Agreement (Must be submitted before any other procedures can be performed)
This contains necessary agreement clauses for carrying out experiments at MLF.
The Users Office will not be able to accept any advance procedures regarding the experiment (including experiment participants) until the Principal Investigator submits this document.
b: Letter of consent (Mandatory for proprietary experiments)
This is the agreement regarding the charge for proprietary experiment use.
The Users Office will not be able to accept any advance procedures regarding the experiment (including any experiment participants) until the Principal Investigator submits this document.
Proprietary experiment users must submit this document with the Letter of Agreement.
c: Notification of Intended Experiment Participants (Mandatory)
Please register all participants in the experiment.
This document needs to be re-submitted if adding or removing participants.
d: List of samples and chemicals used in MLF (Mandatory)
Please submit this document no less than 2 weeks before the experiment if bringing in samples and/or chemicals.
This will be accepted or rejected after a safety review.
e: List of experimental equipment to be brought to MLF (Mandatory)
Please submit this document no less than 2 weeks before the experiment if bringing in equipment.
This will be accepted or rejected after a safety review.

*Equipment subject to a safety review cannot be brought into J-PARC without passing the safety review.

Guideline of experimental equipment requiring a safety review

  • Equipment with a generally high risk or for which safety cannot be confirmed, such as equipment with a high-voltage power supply, modules, or self-made devices.  Machines to be connected to MLF by wiring and/or piping.
  • A safety review is not required for manufactured goods such as laptop computers, digital cameras, and digital voice recorders that will be used for their intended purpose. However, manufactured goods to be used as samples must be included in the document "List of experimental equipment to be brought to MLF".

Step 1. Log in to the User support system.

"Forms for the Spokesperson" is displayed in the menu on the left side of the screen when a principal investigator logs in.

*These forms can also be submitted by a delegate project member preassigned by the principal investigator via the User Support System. (See below for information on how to assign a delegate.)


 

 

Step 2. Click "1. Forms" to get the appropriate forms for your experiment

Click "1. Forms" to get the appropriate forms for your experiment, fill them out and submit them at least 2 weeks before the start of your experiment.

Please select from the menus below the Project Type and Experiment/Proposal Number for
which you wish to lodge forms then click the [Search] button.
Click the [Clear] button to reset the entry fields.

 

 

Step 3. 【Online Application forms】is shown

 

 

 

Step 4. Please submit the documents listed.

Click Generate button to open the form. Marked as Yes to Required is mandatory.

 

The Users Office may send the Principal Investigator a "Request for Approval of User Registration" when an applicant who is not on the member list of the proposal application form applies for User Registration. The "Request for Approval of the User Registration" will be sent to the Principal Investigator automatically from the User Support System. Please log in to your User Portal, confirm the applicant's information and give approval or rejection.

Step 1. Log in to the User support system.

Click Team Member Approval at menu.

 

Step 2. Search the purpose and fine the name you wil approve.

 

Step 3. Tick the person

 

Step 4. Click Details

To check the registered details and process the Approval Status of individual applicants, please select the applicant from the table (Individual Select) and then click the [Details] button. 

Step 5. Completed

 

 

Delegate Principal Investigator

An experiment participant who behave as same as the Principal Investigator.
There are 2 ways to register a delegate Principal Investigator.

 

NOTE

・Delegate Principal Investigator must be a participant of experiment
・Students cannot be delegates.
・A few people can be delegate Principal Investigator

 

The Principal Investigator performs the registration. To do this, the assigned delegate Principal Investigator needs to have been registered as a member of the experiment to the User Support System beforehand.

 

Operate by the Users Office

・The Principal Investigator attaches a comment specifying the delegate Principal Investigator when submitting the experiment participant list.
・ The Principal Investigator submits a written request stating the name of the assigned delegate to the Users Office.

 

Operate by the Principal Investigator

 

Step 1. Log in to the User Support system and click [Forms for the sporksperson] in the left menu.

Click [2. Team Member Authority Management] to locate the applicable experiment number.

 

 

Step 2. Select from the Project Type and Experiment/Proposal Number

Please select from the menus below the Project Type and Experiment/Proposal Number for
which you wish to lodge forms then click the [Search] button.
Click the [Clear] button to reset the entry fields.

 

Step 3. Change the level of authority of the applicable person from “Participant” to “Delegate Principal Investigator”

 

 

Step 4. Click [Confirm] button

 

Step 5. Completed
